There are numerous options available. Type in "survey software" in your favorite search engine and you'll find a veritable buffet of programs.
I have used Zoomerang and it worked very well. They have a free-limited feature membership which allows to use and test the system for yourself. A couple other similar services which came up in my research were QuestionPro and Surveyz.
If it turns out that you are as good as you think you are, this will be more ammunition when it is time for your next personal or departmental review.
